Observed

Measured during or after the sprint, but not yet independently verified. It's a data point with a source — not yet a proven outcome.

Verified

Confirmed by an independent reviewer against delivery documentation. The outcome, the basis, and the reviewer's identity are all recorded. This is the only label we call proof.
